Somerset & Kennebec Counties Community Partnership (SKCCP)

The Somerset & Kennebec Counties Community Partnership (SKCCP), led by Healthy Living for ME/Central Maine Area Agency on Aging in Augusta, Maine, is dedicated to building coordinated, equitable systems that promote healthier communities across Somerset and Kennebec Counties. Through a HRSA Rural Health Network Development Planning Grant, SKCCP is improving collaboration between healthcare and community-based partners to better serve individuals with intellectual and developmental disabilities (IDD), Alzheimer’s disease, dementia, and their caregivers. The initiative focuses on strengthening communication, expanding access to services, and developing a sustainable framework for long-term health equity. Guided by its steering committee and informed by local stakeholders, SKCCP has translated broad goals into tangible, actionable strategies and a strategic plan that emphasizes programmatic impact, partner engagement, and network sustainability.

Downeast Housing Collaborative - Hancock and Washington Counties

The Downeast Housing Collaborative, led by Healthy Acadia, unites over 20 cross-sector partners across Hancock and Washington Counties, Maine, to address homelessness and housing insecurity as a key determinant of community health. By fostering collaboration among providers, organizations, and residents, the Collaborative has developed and implemented a regional plan that strengthens systems, expands access to housing across the continuum—from emergency to moderate-income—and centers community voices, especially those with lived experience. Through trust-building, shared vision, and innovative local solutions such as housing navigation, voucher-based units, and landlord liaison programs, the Collaborative continues to improve housing stability and health outcomes in the Downeast region.
