Regional Collaboration
Purpose and Target Population
Penobscot Bay Community Health Partnerships (PBCHP), the grant-funded Community Health Department of the Penobscot Bay YMCA, serves the rural communities of Knox and Waldo Counties in Midcoast Maine.
PBCHP works collaboratively with schools, healthcare providers, municipalities, recovery organizations, businesses, and community partners to reduce youth substance use, strengthen protective factors, reduce stigma surrounding substance use disorders, connect individuals and families to recovery resources, and improve community health.
Through evidence-based prevention education, coalition building, environmental prevention strategies, tobacco prevention, free community nutrition education, and harm reduction initiatives, PBCHP helps communities build healthier environments where individuals and families can thrive across the lifespan.
Planning and Implementation
PBCHP's work is guided by local data, including the Maine Integrated Youth Health Survey, community needs assessments, and ongoing input from coalition partners. This data-driven approach ensures community health efforts remain responsive to local priorities while allowing partners across sectors to coordinate strategies rather than work in isolation.
Programs include evidence-based substance use prevention education for middle and high school students in traditional public schools as well as alternative learning environments like the Ecology Learning Center and Wayfinder Schools.
Through its Nourish ME program, PBCHP provides free nutrition education, hands-on cooking skills workshops, and food literacy resources for children, families, and adults across the region.
PBCHP also supports municipalities, schools, employers, and healthcare organizations through policy technical assistance, tobacco and vaping prevention, state-certified Responsible Beverage Seller/Server trainings, overdose response training, and the distribution of free safe-storage supplies.
PBCHP also works to reduce stigma surrounding substance use disorders by providing community education, partnering with recovery organizations, connecting individuals to local recovery supports, and promoting compassionate, evidence-based approaches to prevention and recovery.
Best Practices
PBCHP’s work is grounded in the strength and commitment of its regional coalition. Convened through the Penobscot Bay YMCA, this collaborative network brings together local schools, healthcare systems, municipalities, law enforcement, recovery partners, and community agencies across Knox and Waldo Counties.
By coordinating strategies and sharing resources through a unified framework, coalition partners are able to break down silos, maximize limited rural resources, and respond collectively to emerging community needs.
The coalition intentionally brings together prevention professionals, healthcare providers, recovery organizations, municipalities, schools, businesses, and individuals with lived experience to strengthen collaboration across the full continuum of prevention, harm reduction, treatment, and recovery.
Challenges and Solutions
Like many rural regions, Knox and Waldo Counties face geographic barriers, limited staffing capacity, transportation challenges, and fewer prevention resources than more urban areas.
PBCHP addresses these challenges by leveraging strong cross-sector partnerships, sharing resources, and bringing prevention and nutrition programming directly into schools, town centers, and community spaces.
By coordinating regional efforts rather than relying on individual organizations to work independently, PBCHP maximizes limited resources while expanding access to vital public health services across diverse rural populations.
PBCHP's work is supported through a diverse portfolio of federal, state, and local grants, allowing the partnership to integrate prevention, nutrition, tobacco prevention, and community health initiatives under one coordinated regional framework.
Resources and Tools
Resources include evidence-based prevention curricula, coalition planning tools, free community nutrition education and recipe guides, monthly parent/educator newsletters, the Maine Integrated Youth Health Survey, state-certified Responsible Beverage Seller/Server training materials, Reentry backpacks, and practical safety supplies including medication lock boxes, Deterra® drug deactivation pouches, cannabis safe-storage locking bags, and Maine QuitLink materials.
